Clinician's Guide to Medications for PTSD

Paul Holtzheimer, MD and Macgregor Montaño, PharmD, BCPP

The VA/DoD Clinical Practice Guideline (CPG) for PTSD recommends specific trauma-focused psychotherapies as first-line treatments for PTSD over pharmacotherapy (1). Medications are an effective treatment option when these trauma-focused psychotherapies are not available or feasible when patients prefer medications. PTSD also carries high levels of psychiatric comorbidities, such as depression, which may be treated with medication.

Medications prescribed for PTSD act upon neurotransmitters affecting the fear and anxiety circuitry of the brain including serotonin, norepinephrine, g

Traumafokuserad kognitiv beteendeterapi (TF-KBT)

What Type of Treatment Is This?

SSRIs (selective serotonin reuptake inhibitors) and SNRIs (serotonin-norepinephrine reuptake inhibitors) are types of antidepressant medication. Certain antidepressants can reduce PTSD symptoms.

Medications have 2 names: a brand name (for example, Zoloft) and a generic name (for example, sertraline). There are 2 SSRIs and 1 SNRI that are recommended for PTSD:

  • Sertraline (Zoloft) - SSRI
  • Paroxetine (Paxil) - SSRI
  • Venlafaxine (Effexor) - SNRI

There are other types of antidepressant medications, but the 3 medications listed above are the ones that are most effective for PTSD.
