The VA/DoD Clinical Practice Guideline (CPG) for PTSD recommends specific trauma-focused psychotherapies as first-line treatments for PTSD over pharmacotherapy (1). Medications are an effective treatment option when these trauma-focused psychotherapies are not available or feasible when patients prefer medications. PTSD also carries high levels of psychiatric comorbidities, such as depression, which may be treated with medication.

PTSD diagnostiseras ofta inom primärvården. Det finns inget som hindrar att behandlingsförsök sker i primärvården om berörd vårdpersonal har nödvändig kännedom om behandlingen. Vid otillräcklig behandlingseffekt eller svårare former av PTSD (hög grad av samsjuklighet och låg funktionsnivå) bör patienten dock remitteras till specialistpsykiatrin.
Samsjuklighet t ex depression kan behöva behandlas innan de specifika behandlingsmetoderna mot PTSD kan tillgripas. Vid samtidigt skadligt bruk/beroende bör behandlingen integreras med insatser mot alkohol- eller narkotikaproblemen.

Traumafokuserad kognitiv beteendeterapi (TF-KBT)
Traumafokuserad kognitiv beteendeterapi (TF-KBT) är den behandlingsmetod som har störst vetenskapligt stöd och något som patienter med PTSD och cPTSD i första hand bör erbjudas.
I TF-KBT exponeras patienten upprepat för t ex minnesbilder, situationer, känslor eller fysiska sensationer kopplade till traumat samtidigt som de undvikande beteendena inskränks. På det sättet uppnår patienten med tiden habituering till yttre och inre triggers, korrigering av ohjälpsamma föreställningar kring traumat och i förlängningen en gradvis

What Type of Treatment Is This?
SSRIs (selective serotonin reuptake inhibitors) and SNRIs (serotonin-norepinephrine reuptake inhibitors) are types of antidepressant medication. Certain antidepressants can reduce PTSD symptoms.
Medications have 2 names: a brand name (for example, Zoloft) and a generic name (for example, sertraline). There are 2 SSRIs and 1 SNRI that are recommended for PTSD:
- Sertraline (Zoloft) - SSRI
- Paroxetine (Paxil) - SSRI
- Venlafaxine (Effexor) - SNRI
There are other types of antidepressant medications, but the 3 medications listed above are the ones that are most effective for PTSD.
